NOTICE OF NAME CHANGE BY MERGER

Welcome to MIDFLORIDA Credit Union! On March 1, 2017, Martin Federal Credit Union merged with MIDFLORIDA Credit Union. The credit unions’ computer systems, forms, fee schedules and contractual documents will not be fully integrated for several months, and you may be asked to sign documents, or you may receive notices and disclosures, in the name of Martin Federal Credit Union after the merger.

• On any document dated or effective on or after March 1, 2017, “Martin Federal Credit Union” will mean “MIDFLORIDA Credit Union”;
• any new accounts opened on or after March 1, 2017 are opened with MIDFLORIDA Credit Union as the financial institution;
• any new loans opened on or after March 1, 2017 are owed to MIDFLORIDA Credit Union as the lender;
• accounts and loans existing prior to March 1, 2017 will continue to be subject to the same terms, conditions and fee schedules until such time as you receive notice from the credit union that the terms, conditions or fee schedules have changed;
• accounts and loans opened on or after March 1, 2017, may be subject to the same terms, conditions and fee schedules previously published by Martin Federal Credit Union until the computer systems are fully integrated and you receive notice from the credit union that the terms, conditions or fee schedules have changed;
• some accounts may be selectively converted by the credit union to the MIDFLORIDA Credit Union system, terms, conditions and fee schedules, prior to or later than other accounts;
• you will continue to perform your obligations under any agreement with Martin Federal Credit Union to the benefit of MIDFLORIDA Credit Union;
• MIDFLORIDA Credit Union will perform the obligations of Martin Federal Credit Union under any agreement with you;
• You will continue to make any payments to the same address and under the same account number as stated in your agreement, but payable to MIDFLORIDA Credit Union

To change your mailing address, e-mail address or to update your phone numbers, please complete this form and fax it to 407-210-1892.

To set up online banking to transfer between accounts that you are associated with, complete this form and fax it back to 407-210-1892.

Please complete this form for all signature based debit card disputes. Once completed, please fax to 407-210-1895.

Please complete this form along with the Debit Card Dispute Form related debit card disputes. Once completed, please fax both forms to 407-210-1895.

Do you need a new or replacement debit card? Complete this form and fax to 407-210-1892.

To help your employer set up direct deposit for you, please print this form and take it to your company payroll department.

Please have all parties associated with your account fill out this form and turn it into one of our branches. All signatures on this form must be notarized or it will be considered invalid and you will be required to fill it out again.
